The Scheduling-Intake Coordinator is responsible for managing the patient intake process, including communicating directly with patients and families, data entry, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, participating in a daily intake stand up meeting, monitoring portals and managing the insurance verification and authorization processes.
DUTIES & RESPONSIBILITIES
- Monitors partner portals for incoming referrals.
- Carries out daily patient referral and intake operations including implementation and execution of intake best practices.
- Ensures compliance with all state, federal, and ACHC referral/intake regulatory requirements.
- Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
- Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
- Maintains comprehensive working knowledge of Home Health contractual relationships and ensures that patients are admitted according to contract provisions.
Process authorization & scheduling notes in WellSky (Kinnser)
- Compare submitted communication notes with physician orders
- Plot and schedule visits
- Work with field team to find staffing coverage
- Collaborate with the utilization review and authorization teams
Qualifications and Skills:
We use Wellsky Kinnser EMR and other modern tools to make our Intake process modern.
One year of scheduling/registration or other related field
- Excellent verbal and written communication skills
- Knowledge of medical terminology a plus
- Experience with EMR systems WellSky (Kinnser) is a must have
- Home health scheduling experience is must have
- Overall knowledge of business office procedures
- Computer data entry skills
- Strong clerical & organization skills
- Ability to work with others in a flexible and cooperative manner
