Performs intermediate-advanced human support work providing information and assistance for follow-up of crisis services received; assisting in finding options and connecting and enrolling them in public and private program and services; providing state-mandated legal services as defined by state statutes; managing Emergency Mental Health Service Programs; coordinating follow up care and case management for Behavioral Health and Substance Use Disorders emergency detention and hospitalization; reviewing and monitoring crisis reports; providing crisis counseling and intervention, and related work as apparent or assigned. Work is performed under the limited supervision of the Behavioral Health Manager.

Examples of Duties

  • Utilizes a trauma-informed approach to provide person-centered crisis treatment, recovery, options counseling, and support services to clients in behavioral health and long-term support programs.
  • Coordinates and attends probable cause hearings with the circuit court.
  • Develops and distributes client crisis plans, safety plans, or behavioral support plans.
  • Serves as liaison between inpatient facility staff and agency in developing aftercare and crisis plans, coordinates the provision of services, monitors service delivery, and maintains linkage with the court.
  • Reviews, monitors, and manages the Northwest Connections (NWC) portal and reports.
  • Prepares the monthly crisis calendar with the mobile on-call team and sends the completed calendar to NWC.
  • Organizes monthly staff and community crisis meetings.
  • Provides crisis intervention work at the Adams County Jail as required.
  • Responds to clients in crisis or emergency situations at the agency or in the community; may have to respond to crisis or emergency calls after normal business hours.
  • Participates in the after-hours mobile and on-call system.
  • Monitors and coordinates the legal aspect of case management.
  • Maintains client records in accordance with regulatory guidelines.
  • Provides accurate and timely case management documentation.
  • Serves as point of contact for the Health and Human Services Department relative to assigned external agencies, programs, or services and represents the department at professional meetings and community events as required.
  • Consults and receives supervision from other professional staff within the agency.

Essential Qualifications

Bachelor's degree in social work, human services, or related field and moderate experience in legal, administrative, and governmental operations, child and family support, or equivalent combination of education and experience. Valid driver's license in the State of Wisconsin.

Thorough knowledge of County and department policies and procedures to include emergency and inclement weather policies; ability to prioritize and manage multiple, concurrent projects; attention to detail and ability to effectively organize time and anticipate, plan and successfully respond to changing circumstances; thorough knowledge of the legal aspect of case management; ability to meeting with inmates at the jail; ability to respond to crisis calls; ability to make arithmetic computations using whole numbers, fractions and decimals; ability to operate and use standard office equipment; knowledge and ability to use standard office and position-specific software; ability to effectively communicate both verbally and in writing; ability to establish and maintain effective working relationships with co-workers, supervisors, other departments and the general public.

PHYSICAL DEMANDS

This work requires the occasional exertion of up to 10 pounds of force; work regularly requires using hands to finger, handle or feel and reaching with hands and arms, frequently requires sitting, speaking or hearing and repetitive motions and occasionally requires standing, walking, climbing or balancing and lifting; work has standard vision requirements; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly; hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; work requires preparing and analyzing written or computer data, visual inspection involving small defects and/or small parts, operating machines, operating motor vehicles or equipment and observing general surroundings and activities; work occasionally requires exposure to outdoor weather conditions; work is generally in a moderately noisy location (e.g. business office, light traffic).
