Job description
The Licensed Practical Nurse provides and directs clinically appropriate patient care to an individual or group of patients under the direction and supervision of the Registered Nurse.
In the execution of job duties, it is the universal expectation that all tasks are performed with a patient centered focus, while also seeking opportunities to continually improve core processes. Incumbent will be scheduled based on operational need, which may include but is not limited to: holidays, extended shifts, night and/or weekend shifts, standby and/or on-call. Reports to: Director of Nursing or Resident Care Director
Essential Functions
Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
Performs plan of care interventions, including specimen collection and wound care. Demonstrates proper infection control practice and adheres to Infection Control Policies and Procedures and OSHA standards. Adheres to medication pass protocol and meets regulatory guidelines.
Evaluates nursing care provided by continual review of the effectiveness of the nursing care plan. Collaborates with supervising nurse to make appropriate revisions to achieve the desired outcomes.
Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Works as an advocate for the physical and emotional well-being of the patient.
Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families. Collects and documents patient information as needed (including weights and vital signs).
Provides patient and family education in regard to the hospitalization/admission, medications, and/or in preparation for discharge. Upon discharge, provides appropriate discharge instructions to include follow-up and referral needs.
Provides accurate and timely hand-off communication. Serves as clinical and administrative resource to staff, residents, and families regarding nursing services and procedures, aging processes, medications, outside medical services
Minimum Education
X High School Diploma-Required
Minimum Work Experience
Preferred - 1 Year Long Term Care Experience.
Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
