Responsibilities may include the following but are not limited to:
Traveling to patient's home and providing "Best Practice" nursing care
Checking vital signs
Documenting in a complete and confidential manner
Evaluate the patient’s wellbeing, reporting any needs to the Director of Clinical Nursing or emergency services
Providing personal care and meals
More advanced nursing such as chest tubes, IVs, colostomy, urinary catheter management are also available
Giving emotional support to client and families in times of crisis
Requirements:
Valid LPN license
Annual physical within one year
PPD or Quantiferon within one year or chest x-ray within five years
MMR labs
COVID vaccine card with booster
