A respected client of Alvita Care is seeking experienced, full-time LPN's in Queens to provide 1:1 care for their patients.

Reporting to the Coordinator of Care, the Licensed Practical Nurse will be responsible for seeing patients in their home for revisits based on a skilled need, implement the plan of care and evaluate the treatment. You will document the visit electronically and completing documentation within 24-48 hours.

Requirements:

• Must possess a valid, non-expired NY State LPN License
• Minimum of ONE year Med/surg experience in a hospital or Long-Term Care facility required
• Must have a NY State Drivers License and a car
• Good computer skills required
• Must be available to work the following weekly schedule: Monday to Friday from 9:00am to 5:00pm
