Responsibilities:
The Brookdale at Home LPN will operate within the scope of their nursing license as well as the specific home care license state requirements and physician orders and will be responsible for BAH Home Care services, including but not limited to medication setup, monitoring and medication administration, simple wound care, related paperwork, medication management, supervisory visits and coordination of care and communicating with physician and family.
Provides care to clients in accordance with physician orders, recognized standards of practice and established company policies and procedures.
Assists in maintaining a physical, social and psychological environment in the best interests of clients. Ensures client rights are maintained at all times.- Monitors clients within state licensure regulations; supervises and directs nursing assistants and home health aides where allowed by state licensure regulations.
- May assist in writing care plans based on client needs and change of condition.
- Administers medication and treatments as prescribed by physician or other eligible licensed practitioner such as NP or PA according to state regulations. Performs medication box setups and medication management. Documents and reviews medication and treatment lists for accuracy and compliance with physician orders, professional standards, federal and state regulations and company policy.
- Monitors responses to treatment plans and physician orders. Possesses knowledge of medication interactions and notifies Nursing Supervisor or designee of any issues. Adheres to company systems, processes and procedures.
- Counts controlled substances with off going nurse or designee; secures and corrects reconciliation of controlled substances during shift.
- Assists with discharge process relevant to medication and treatment orders.
- Possesses knowledge of OSHA regulations, infection control standards, and emergency procedures.
- Follows policies and procedures to ensure client safety.
- Documents all pertinent information regarding nursing care, care plans and observation of client overall condition.
- Attends new hire and annual training to maintain skill level and remain current in emerging healthcare trends.
- Maintains licensure.
- Notifies BAH Leader of any client concerns.

Education and Experience
Education as required to obtain state nursing license (LPN/LVN) and/or one to two years related experience and/or training; or equivalent combination of education and experience.
