Oaks at Avon is hiring Licensed Practical Nurses to deliver care to residents or patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.

SUMMARY OF POSITION: The Licensed Practical Nurse is responsible for delivering care to residents/patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with residents/patients, family members, and other health team members while maintaining standards of professional nursing.

CERTIFICATES, LICENSES, REGISTRATIONS:

• Current Clear/ Active State LPN License
• CPR certification

Weekend Warrior: BAYLOR PROGRAM

o The Weekend Warrior program can pay up to $2000.00 a month to a nurse who works weekend shifts only.

o Work 4 scheduled weekend shifts in a 1-week period; receive $500 monthly bonus.

o Work 8 scheduled weekend shifts in a 2-week period; receive $1000 monthly bonus.

o Work 12 scheduled weekend shifts in a 3-week period; receive $1500 monthly bonus.

o Work 16 scheduled weekend shifts in the month; receive $2000 monthly bonus.

Guidelines

o Employee must work the minimum four (4) weekend shifts in the same weekend (Fri, Sat or Sun).

o Weekend Warrior shifts start Friday 3pm and end Sunday 11pm.

o Shifts to be assigned by the staffing needs of the facility.

o Excessive missed days will result in disqualification from the Weekend Warrior program.
