Description/Job Summary

H.E.A.R.T Services is looking for an LPN Full Time and Part Time!

Specific shifts include: Tuesday and Thursdays from 9:00am to 5:00pm or 8:00am to 4:00pm


The LPN (Program Nurse) performs all the medical case management functions for the residents at H.E.A.R.T Services, a program servicing the needs of the mentally ill adults with other acute medical conditions. The nurse ensures continuity of care and provides medical advising support to the Program Director and H.E.A.R.T Services staff. The nurse is responsible for the medical department of the program. The nurse ensures that treatment recommendations are geared towards the overall wellness of HEART Services participants. The nurse ensures that all staff adheres to the medication, medical, psychiatric appointments and interventions prescribed by providers.

3000.00 Sign on bonus and retention bonus available!

  • of approved amount paid at successful completion of 90 days employment, of approved amount paid at 180 days of successful employment*
  • The sign on bonuses pro-rated for Part Time positions being filled. (i.e. 20 hrs/week =50% of the above)
  • On Call staff who take a full-time position are eligible for the above

Responsibilities/Duties

Responsibilities of the job include, but are not limited to the following:
  • Screening of new residents
  • Assess the nursing needs and appropriateness of new intake
  • Discuss all medical and psychiatric concerns of potential residents with Program Director
  • Provide detailed written nursing assessment on all potential residents
  • Ensure that new intake documentations are consistent with state and local licensing requirements prior to admissions.
  • Attend clinical team meetings to discuss and monitor residents' progress towards their goals and objectives.
  • Participate in concurrent reviews
  • Participate in monthly linkage meetings
  • Participate in weekly management team meeting
  • Participate in monthly clinical meetings and facilitate medical case review
  • Document and review nurses note and medical interventions with Program Director
  • Perform and document weekly and routine physical assessments (Including vital signs) in QuickMar
  • Provide ongoing training to residents, families, and other identified support person on resident's medication and medical interventions
  • Monitor and assist with administering medication to residents
  • Conduct weekly medication count discuss findings with the Site Supervisor for program protocol and policy compliance
  • Conduct monthly system review and report findings to the Program Director
  • Maintain the confidentiality of all residents, including addressing medical and psychiatric needs in a closed and secure setting

Required Experience

Minimum 2 years experience required.

Required Education

High School Diploma or GED

Required Qualifications

Certification as a Licensed Practical Nurse

A Valid driver's license

RHD's HEART Services is a licensed (Community Rehabilitation Residence) residential program that utilizes evidence based person-driven practices to provide services needed for individuals to maintain independence in the community.

RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within 8 weeks of hire (start date). This currently does not include boosters.
