Job description

We're currently in search of a Full-time MDS Coordinator for our skilled nursing facility.

The MDS Nurse is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care, as well as completion of LTCMI (Long Term Care Medicaid Information), or LOC (Level of Care) forms for the Medicaid payment process when applicable.

Qualifications:

• Current Texas nursing license (RN or LVN)
• Requires knowledge for working Minimum Data Sheets for all payers
• Minimum 1 years experience as an MDS/PPS nurse in LTC
• Knowledge of PCC is a plus ( Point Click Care )
• Ability to work well with others
