Day to Day Life at Advanced

• Assesses phycological status of patient and social and emotional factors related to the patient's illness, need for care, and response to treatment.
• Problem identification and goal setting.
• Provides direct social casework services including short term individual counseling, community resource planning and crisis intervention to patients and families.
• Assists with goal setting, counseling, planning for discharge, providing interventions and helping with other concerns.
• Advises agency personnel, patients, and families of appropriate community resources, their eligibility criteria, and regulations.
• Facilitates effective referrals of patients and families to appropriate community resources including follow up to see that goals of referral were accomplished.
• Participates in the development and periodic renewal of the patients POC.
• Adheres to the POC.
• Observes and reports to the physician the response of the patient to treatment and changes in condition.
• Maintains verbal and/or written communication with other personnel involved with care of the patient.
• Maintains appropriate and timely patient care records including observations, treatments, patient response to treatment, and changes in patient's condition.
• Attends and participates in appropriate interdisciplinary coordination activities including patient care conferences, staff meetings, and agency committee meetings.
• Provides continuing education and consultation to other staff members including understanding the social and emotional factors related to health problems.
• Provides supervision to social work assistants including case assignment, development of social work treatment plan, periodic supervisory home visits, chart review, and other supervisory functions.
• Evaluates effectiveness and outcomes of care.
• Practices standard precautions.
• Participates actively in the Agency's Performance Improvement Program.
• Performs other related functions as required.

Required Experience

• Currently Licensed in the State of MIchigan to practice as a Social Worker.
• Valid and current driver's license.
• Reliable transportation.
• Thorough knowledge of social work principles and methods.
• Knowledge of individuals and group behavior and ways of working effectively with adults, children and families who have social, economic, emotional, or health problems.
• Knowledge of community organizations and resources and referral procedures.
• Knowledge of psychiatric and therapeutic interventions and techniques suitable to treatment programs.
• Ability to exercise independent professional judgment in complex situations.
• Ability to establish and maintain effective working relationships with patients and families, staff members, outside agencies and institutions, and the general public.
• Ability to communicate effectively both orally and in writing.
