Mission: Psychiatric rehabilitation program (PRP), promotes recovery, for community integration and improve quality of life for persons who have been diagnosed with any mental health condition that seriously impairs their ability to lead meaningful lives.
Psychiatric Rehabilitation services are collaborative, person director and individualized. PRP focus on helping individuals develop skills and access resources needed to increase their capability to be successful and satisfied in the living and working learning and social environments of their choice. More pacifically PRP is desice or eliminate the maladaptive Behaviors that stem from the presence of one or more mental health diagnosis in minors and adults.
Psychiatric Rehabilitation has a direct correlation to psychical Health rehabilitation and how it affects the human body and overall functioning. It is important when attempting to overcome a physical condition prohibiting proper functioning of particular body parts to improve body functions through physical health rehabilitation, likewise in psychiatric rehabilitation it is also important to him individuals mental stability and functioning.
PRP Services with the help of family and others involved in the clients care establish pacific individual rehabilitation goals target to improving each client improper behaviors and functioning stemming from their diagnosis which included but limited to ADHD, bipolar, adjustment disorder, depression disorder, anxiety, and oppositional defiance.
Helping Hands PRP INC provides blended PRP services, to include both on and off site visitation.
Job Duties:
> Complete four (4) visits each month for at least a minimum of one (1) Our for minors. Complete Six (6) Visit each month for a minimum of 30 minutes for adults
> Blended Services- Require on site (Office visitation) & Offsite (Home / Community Visitation)
> On-site- (Office Visitation Only)
> Offsite- (Home/ Community Visitation Only)
> Complete HHPRP On-site notes related two behaviors observed and interventions applied for services rendered to clients.
> Complete monthly notes at the end of every month recording goals addressed, and clients progress to ensure service delivery is related to clients persisting problems and overall mental health needs.
> Completed individual rehabilitation plans for clients every 90 days (review).
> Engage in supervision on a monthly basis with direct supervisor.
> Attend staff meetings and trainings on a monthly basis and as scheduled by direct supervisor work program director.
> Collaborate with therapist in document communication on a monthly basis for continuing of goals develop for each client.
> Direct your staff will complete a medical care collaboration form every six months, documenting The concerns reported by client/family/doctor/mental health clinician in any referral process to be logged in the clients file and will be monitored by the direct supervisor and quality director.
> Collaborate with other individuals pertinent to clients overall care (Parent, guardians, and/or teachers etc.) to monitor behavior and goal attainment on at least a bi-weekly basis.
> Provide guidance to clients regarding vocational, educational, family, social, and overall daily life choices.
> Active participation in linking clients to resources.
Intake Specialist:
> Determine clients eligibility in needs for rehabilitation services. ( intake/ initial Screening)
> Analyze & Interprets the pertinent Educational, Social, Medical, Psychological & Behavioral Information relative to the client in order to determine level of aspiration, interests, motivation, and aptitude related to the client achieve and goals.
> Develop comprehensive individual rehabilitation plans for clients within the first 30 days ( initial IRP)
Minimum Qualifications
> Must have at least one year of applicable experience working with adults and minors
> Must be years old
> Unless have a high school diploma
Travel Requirements: PRP counselors use personal vehicle for all appointments with clients. Counselors must be able to travel to in person supervision appointments as well as other require events, meeting, and activities. Telehealth is temporary encouraged due to COVID-19.
In accordance with (COMAR 10.21.29..09), so HHPRP counselors must have 60 hours of orientation/training/shadowing before providing service to adults in my nose service by the program. These hours must be completed within a two-week period and will be structured by direct supervisor. Temporarily providing telehealth services due to COVID-19.
