Position Summary:

A Mental Health Worker/Case Manager will assist in the development of treatment plan(s) that are appropriate to the needs and desires of the consumer. The treatment plan(s) will aim to promote recovery and stability for community living and helping the client to achieve higher levels of function.

Core Duties And Responsibilities:

• Monitor consumers compliance with treatment as prescribed by treatment team.
• Develop individualized case management treatment plan(s) with the consumers in the Children/Adolescent Outpatient Program.
• Assist with establishing coordination with local agencies in the provision of services.
• Assist with the development and maintain resource directory for agency.
• Update and maintain consumer records.
• Arrange for medical care-exams, emergency shelter, food, and/or education services for consumers at large.
• Facilitate support/self help groups for consumers, family members and the community at large.
• Participate in case disposition meetings on a routine basis with the assigned treatment team.
• Provide and document services in accordance with the Rehabilitation Option/Targeted Case Management requirements.
• Participate in the Mental Health education program provided to the community.
• Coordinate consumer care with other providers both within and outside the agency.
• Attend and participate in all in-service training as necessary.
• Responsible for timely completion of documentation in the client?s clinical record per Enki, DMH and legal requirements.
• Responsible for meeting assigned minimum productivity requirements on a monthly basis.
• Comply with all Company policies and procedures as applicable.

Minimum Education And Experience Preferred:

• High school diploma plus two (2) years experience in a mental health setting; or BA Degree from accredited college/university in a health related field (i.e. social work, psychology or sociology).

Licenses/Certifications:

• Valid California driver?s license, current automobile insurance, and eligibility to drive for the Company.

Knowledge, Skills And Abilities:

• Ability to perform in a positive, courteous, flexible and adaptive manner.
• Able to exercise independent judgment and initiative.
• Computer literate and experienced with computer programs such as Microsoft Office Word and Excel.
• Experience using TIER and/or other Electronic Medical Record databases is a plus.
• Must be able to communicate clearly, both orally and written.
• Bilingual English/Spanish preferred but not required.

Physical Requirements:

• See well enough (with or without corrected vision) to read fine print, hand-written materials, and a computer screen.
• Speak and hear well enough (with or without assistive device) to converse in person and over the telephone.
• Finger, handle, feel, and reach with hands and arms, including ability to use a computer keyboard.
• Have the mobility (with or without assistive device) to stand, sit, crouch, and move from one work area to another.
• Have the strength to lift, carry or push office equipment and supplies used in normal job functions.
• Stamina to effectively perform work activities.
• Must be able to lift 15-20 lbs.

Pay Range: $17.00-$28.00/Hour (Determined by Experience and Qualifications)
