Job Description

The Nurse Navigator plans, coordinates and delivers health services to patients receiving care within the defined Service Line. This includes working collaboratively with other health providers to assure coordinated care throughout the service line's continuum of care. The nurse navigator will assist with the coordination of services and will provide support and education throughout diagnosis, treatment and into survivorship for patients. Will serve as a professional resource in the areas of the Service Line.

  • Act as a liaison between healthcare providers, payors, and the patient /family.
  • Assist patients with the scheduling of appointments for follow-up visits with other healthcare providers.
  • Contact patients instructing them of additional imaging procedures that must be performed to clarify interpretation.
  • Serve as a consistent contact for patient / family throughout the process of diagnosis and treatment, providing pre-procedure teaching and post-procedure follow-up calls.
  • Work with providers on treatment plans and follow up care
  • Assist with biopsy procedures when requested.
  • Develop, coordinate, and continually improve nursing procedures and processes pertaining to the Service Line coordination of care.
  • Participate in outcomes research.
  • Serve as a resource and consultant for the Service Line as well as the community, developing a presence in the professional and general public.
  • Travel required as needed to Norwich and Mystic offices.

Qualifications

  • Licensed as a professional RN in the state of Connecticut
  • BSN required
  • Minimum of 5 years nursing experience
  • Experience within Neurosurgery Service Line or experience in a multispecialty environment preferred
  • EPIC experience
