We are seeking the "right fit" nurse practitioner for our clinics in Skilled Nursing, Assisted Living and Independent Living Facilities in Denver. This is interesting medicine! If you are an experienced Nurse Practitioner that loves working with the geriatric population, dealing with multiple co-morbidities and functional deficits this is the challenge and rewarding work you are looking for. Coordination of care across providers is key, as is management of care transitions. We have state of the art EMR systems both at the clinic and community level. We are currently seeking an experienced Nurse Practitioner with state certification and licensure in Colorado. We have full-time and part-time openings available.

Nurse Practitioner Job Summary

We are looking for an experienced and dedicated Nurse Practitioner (NP) to join our growing staff. You will provide direct patient care, discussing patient symptoms and concerns and performing thorough examinations to diagnose their medical problem and prescribe medication or create a treatment plan if necessary. When required, you will collaborate with our physicians to treat for more advanced illnesses or injuries. Our ideal candidate has a current state license to practice and at least five years of medical experience.

Nurse Practitioner Duties and Responsibilities

  • Facilitate new patient consultation and health history review
  • Perform physical examinations and screenings of patients
  • Order and administer tests including bloodwork
  • Review and interpret test results to diagnose health problems
  • Recommend further treatment options or future appointments if needed
  • Provide referrals to other practitioners when necessary
  • Provide after-care/ongoing care information and resources to patients and their families
  • Ensure patient records are kept up to date including test results, immunizations, condition changes, etc.

Nurse Practitioner Requirements and Qualifications

  • Master's degree in nursing
  • Current state nurse practitioner license
  • 5+ years of nursing experience
  • Understands HIPAA and OSHA regulations
  • Able to operate as part of a team, alongside physicians and other nurses
  • Proficient in EHR medical charting system and computer-based data management programs and information systems
