This HYBRID position is 50% Remote & 50% in Client Homes!

Full Time: $35-$45/Hr (Depending on Experience & Training) Plus Excellent Benefits.

Job Description:

LifeConnections Specialized Support Services is looking for a Full Time as well as a Part Time (evolving into full time) RN Nurse Trainer to become part of our dynamic team. These positions serve youth (18-21yo) and adults (21+) with Intellectual / Developmental Disabilities (I/DD) living with Home Care Providers in mid-southern New Hampshire (Nashua, Manchester, Concord) and the NH Seacoast. The ideal candidate must have at least two years of nursing experience (any specialty), be licensed in New Hampshire and be willing to become a Certified Nurse Trainer (paid training). No previous experience serving adults with intellectual/developmental disabilities (I/DD) is required. Teach, consult and advocate to help individuals with intellectual/developmental disabilities reach their full potential. Home Visits as well as remote. Corporate office space in Manchester, NH and Hampton Falls. Service hours are flexible but mostly Mon-Fri 9-5. Set your own schedule. Paid Mileage. Laptop provided.

Training

· Medication Administration & Healthcare Coordination per He-M 1201 State Regulations (on –going, as needed and yearly retraining)

· Medical and Health Care Related Protocols (PRN meds, epi pens, g-tube, VNS magnet, supra-pubic catheter, insulin administration, blood sugar checks, vital signs, CPAP, Coumadin, Clozaril, Nebs, Inhalers, wound care, dysphagia, diets)

· CPR, Dementia Care, Safety, Basic First Aid, Blood Borne Pathogens

· In-Service Trainings as needed

Consulting

· Medications and Treatments

· Clinical consults to teams that support health of individuals

· On-Call Rotation every 3 weeks – triage & address health care and medication questions from home care providers, day staff and office staff

· Collaborate with hospital staff when individuals are hospitalized and participate in discharge planning

· PASAAR Evaluations and Clinical Rationales

· Clinical Response Team (CRT)

Leadership

· Attend DDNH meetings monthly

· Attend Staff & Service Agreement meetings as needed

· Continue to improve quality of Nurse Trainer Role

· Serve as liaison to hospital and nursing home nurses, area agencies, other vendors and other agencies we work with

Regulatory

· Conduct Quality Assurance Audits via an on-site review in compliance with state regulations He-M1201 every 1-3 months depending on the setting and ensure compliance with follow up recommendations

· Assist with certification preparation and licensing and follow up as needed

· Train and supervise staff according to rule of delegation NUR 404.04

· Nursing 5 day and 30-day Visits for individuals new to services

· Review and sign Health History within 30 days of admission and Service Agreement yearly

· Self-Med Assessments for all individuals (Yearly for those who self- medicate)

· Ensure progress towards annual health goals via the Annual Health Screening Recommendation form and ensure completion annually

· PRN Protocols are written and signed within 30 days

· HRST (Health Risk Screening Tool) Tracker is completed monthly and in the record (signed yearly)

Please Note:

The bureau administrator or designee shall, upon request, grant designation as a nurse trainer to any nurse licensed in New Hampshire who:

(1) Has 2 years of licensed nursing experience within the past 5 years, at least one of which was as a registered nurse;

(2) Has completed a 6-hour orientation program conducted by the bureau; and

(3) Is not under disciplinary action pursuant to RSA 326-B:37, III.

(4) Has a NH Drivers License in good standing
