We are currently looking for an Occupational Therapist - OT to join our Home Care team!

This Is You:

• Licensed Occupational Therapist - OT in the state of New York with at least 1 year of clinical experience; Home Care preferred
• Oasis experience is strongly desired
• Have reliable transportation and be willing to commute to provide the best in-home patient care
• Willing to work some rotating weekend days required and one summer and one winter holiday.
  
• Willing to take advantage of our excellent training system comprised of in-house and field orientation and become an elite healthcare provider!
• Some weekend days are required, and summer and winter holiday commitments as needed

Together, we:

• Evaluate the patient's functional status and home environment for hazards or barriers to more independent living. Identify equipment needs. Participate in the development of the individualized plan of care
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan
• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system. Submits all clinical documentation, time records and other information in the required time frame
