Summary:
The On-Call Coordinator will support people receiving services from Benedictine to achieve their goals, established according to preferences, abilities and choices, in their Person Centered Plans (PCP). Specifically, the position of On-Call Coordinator will focus on continuity in supports from weekends to weekdays, coordinating follow-up with Program Coordinators, Group Home Managers, Nurses, and Residential Administrators. This position will strive to achieve the mission of Benedictine and operate in a person-centered and ethical manner that always focuses on quality of life and the health, safety and well-being of the people we support.
Essential Duties/ Responsibilities:
Consistently report to work on time as scheduled, prepared to perform assigned duties and responsibilities during entire scheduled work period/shift.
Responsible for weekend on call: resolve problems and maintain effective relationships with residential staff.
Required to cover open shifts on the weekend that occur due to lack of coverage and/or call-outs. If all shifts are covered On-Call Coordinator will work a minimum of one 8 hour shift each weekend, rotating among all 18 adult services group homes, in the capacity of House Counselor.
Communicates every Friday afternoon with Residential Director to get a briefing for the weekend.
Maintains communication with Residential Director, Scheduler, and Group Home Managers as needed to problem solve issues with the schedule.
Arrange for Residential Director, Residential Scheduler and/or Group Home Managers to provide back-up on call as needed.
Work with individuals and staff to achieve goals established in the PCP including providing staff training to understand and implement goals on weekends.
Perform random reviews of weekend documentation maintained by direct support staff and follow-up as appropriate.
If needed, incorporate and follow behavior protocol for each individual as it fits into his/her plan.
Provide written report to Residential Director, Group Home Managers, and Program Coordinators weekly with summary of on call activity and recommendations for follow-up.
Based on knowledge of people supported in residential group homes, provide feedback and recommendations prior to Person Centered Planning (PCP) meetings to support peoples goals and desires for living, working and leisure for the future.
Contribute to the development of Support Parameters document for each person supported, as appropriate.
Advocate with and for the individual to ensure that they are working toward the goals that they choose and achieve the quality of life they desire, balanced with ensuring best possible health and personal safety.

Other Duties/Responsibilities:
Stay current with all required trainings and certifications.
Communicate with Parents, DDA representatives, and Nursing department, as needed.
Assist with the follow-up for incident reports, and other certification and licensing activities as schedule permits.
Cooperate with Benedictine administration and in-state/out-of-state agencies in any inspection or investigation.
Attend and successfully complete mandatory trainings and maintain job-specific certifications/licenses.
Other duties, as assigned.
Job Qualifications:
High School diploma or GED required, Bachelors degree in related field preferred.
Related experience working in human services, specifically with people with disabilities including experience with providing direct support, scheduling, supervising, coordinating resources, individual planning processes, and working with funding agencies highly preferred.
Excellent communications skills and good organization skills required.
Must present professional manner at all times with individuals, families, and colleagues.
Must be Med Tech certified, or be eligible to obtain Med Tech certification upon hire.
Must be a minimum of 18 years old.
Must be able to provide proof of eligibility to work in US upon offer.
Must pass pre-employment drug screen.
Must pass pre-employment criminal background checks through CJIS as stated in Family Law Article 5-561, and pass pre-employment CPS check.
If position requires driving, must have current and valid drivers license in good standing and must have a driving record that would permit coverage under Benedictine drivers insurance.
(If applicable) If position requires use of personal vehicle for transportation service, must be able to use personal vehicle in the execution of job duties, and provide required insurance documentation of personal vehicle.
