Job Summary and Qualifications

This position is designed to assists surgeons with various types of surgical cases. Has the ability to perform all of the job duties of a Scrub Technician and demonstrates competency in first assisting surgeons in all specialty areas. It is required that a First Assistant be a certified OR Technician and have completion of a FA program or became a Certified First Assist through examination by The National Board of Surgical Technology and Surgical Assisting. As defined by the American College of Surgeons (ACS), surgical assistants provide aid in exposure, hemostasis, closure, and other intraoperative technical functions that help the surgeon carry out a safe operation with optimal results for the patient. In addition to intraoperative duties, the surgical assistant also performs preoperative and postoperative duties to better facilitate proper patient care. The surgical assistant performs these functions during the operation under the direction and supervision of the surgeon and in accordance with hospital policy and appropriate laws and regulations.

Role of the Surgical Assistant

The following description of the surgical assistant has been approved by the American

College of Surgeons and Association of Surgical Technologists:

    • Positioning the patient
      • The surgeon shall convey the exact position that will give the best exposure for the surgical procedure. The surgical assistant will carry out this order. Consideration will be given to the patient’s comfort and safety.
      • Points of pressure shall be padded: elbows, heels, knees, eyes, face, and axillary region.
      • Circulation shall not be impaired. (A tourniquet may be required for some procedures.)
      • Nerve damage shall be guarded against.
      • The temperature of the patient should be discussed with the anesthesia personnel and methods employed to maintain the desired temperature range.
      • The surgical assistant shall be familiar with common positions related to the surgical procedure and will be able to use the equipment necessary to provide the position. Competencies will include the following:
        • Fracture tables
        • Head stabilizers
        • Body stabilizers
        • C-arm extensions
        • Any other equipment needed
      • Upon completion of the procedure, the patient shall be evaluated for any possible damage from positioning which will include assessment of the skin. The abnormal condition shall be reported to the surgeon and treatment and documentation shall be carried out.
    • Providing visualization of the operative site by the following:
      • Appropriate placement and securing of retractors with or without padding
      • Packing with sponges
      • Digital manipulation of tissue
      • Suctioning, irrigating, or sponging
      • Manipulation of suture materials (e.g., loops, tags, running sutures)
      • Proper use of body mechanics to prevent obstruction of the surgeon’s view
    • Utilizing appropriate techniques to assist with hemostasis
      • Permanent
        • Clamping and/or cauterizing vessels or tissue
        • Tying and/or ligating clamped vessels or tissue
        • Applying hemostatic clips
        • Placing local hemostatic agents
      • Temporary
        • Applying tourniquets and demonstrating awareness of the indications/contraindications for use with knowledge of side effects of extended use
        • Applying vessel loops
        • Applying noncrushing clamps
        • Applying direct digital pressure
    • Participating in volume replacement or autotransfusion techniques as appropriate
    • Utilizing appropriate techniques to assist with closure of body planes
      • Utilizing running or interrupted sutures with absorbable or nonabsorbable material of wound layers, including muscle and fascia.
      • Utilizing subcuticular closure technique with or without adhesive skin closure strips
      • Closing skin with method per surgeon’s directive (suture, staples, etc
      • Postoperative subcutaneous injection of local anesthetic per surgeon’s directive
    • Selecting and applying appropriate wound dressings, including the following:
      • Liquid or spray occlusive materials
      • Absorbent material affixed with tape or circumferential wrapping.
      • Immobilizing dressing (soft or rigid)
    • Providing assistance in securing drainage systems to tissue
    • Assists surgical team with set up of surgical case to include –pulling case cart and marking preference card and updating if necessary, opening sterile items.
    • Scrubs in and further assists with set up of case if needed.
    • Demonstrates technical proficiency as evidenced by performance
    • Performs first assistant responsibilities requirements.
    • Demonstrated knowledge of surgical anatomy, physiology, and operative technique relative to operative procedures wherein the FA assists.
    • Provides care for different types of patients with varying acuity.
    • Assists in the positioning, skin preparation, and draping of the patient.
    • Provides homeostasis and wound exposure, handles tissue appropriately to reduce the potential for injury, sutures tissue, and applies dressings to the wound under the direction of the surgeon.
    • Uses surgical instruments skillfully in ways consistent with their design and purpose.
    • Other duties as requested by the surgeon that falls within the scope of practice.
    • Contributes to the development of standards of practice.
    • Identifies patient care safety issues and assumes responsibility for appropriate interventions.
    • Demonstrated ability to perform effectively in stressful and emergency situations.
    • Demonstrates skill in behaviors that are unique to functioning as a first assistant.
    • Breaks down case and assists with turnover for next case if needed.

    • Attend Code of Conduct training course annually
    • Demonstrate an understanding of and adherence to HCA’s Code of Conduct

    • Understands and satisfies the needs of the patient population appropriate to the age of the patient served (adults, geriatric, adolescent, pediatric, and parents of neonates)
    • Adheres to the Standards of Behavior for Terre Haute Regional Hospital as per the Customer Relations Policy


Essential educational/licensure, skills and experience:

    • Completed FA program with completion of testing for certification Surgical assistants graduate from surgical assisting programs accredited through ARC/STSA, a collaborative effort of AST, ACS, and SASA, by CAAHEP. CAAHEP is a recognized accreditation agency of the Council for Higher Education Accreditation (CHEA). In addition, surgical assisting programs are located in educational institutions that are institutionally accredited by agencies recognized by the United States Department of Education (USDE), The Joint Commission, or a state agency acceptable to CAAHEP and the ARC/STSA. The ARC/STSA is also a member of the Association of Specialized and Professional Accreditors (ASPA).
    • Certification is conferred by the National Board of Surgical Technology and Surgical Assisting (NBSTSA). Initial certification as a Certified First Assistant (CSFA) is based upon satisfactory performance on the national certifying examination following completion of an accredited program in surgical assisting or another pathway acceptable to the NBSTSA. CSFAs maintain their certification by earning 75 hours of approved continuing education in a four-year period or by successfully retaking the certifying examination at the conclusion of the four-year period. The NBSTSA’s certification program is accredited by the National Commission for Certifying Agencies (NCCA), the accreditation division of the National Organization for Competency Assurance (NOCA) and is in compliance with NCCA’s Standards for the Accreditation of Certification Programs. NCCA standards and accreditation services are referenced requirements in state and federal legislation pertaining to personnel certification. NOCA is accredited by the American National Standards Institute (ANSI) as a developer of American National Standards. ANSI accreditation provides third-party validation that NOCA’s standards development process ensures openness and due process
    • Demonstrate knowledge of surgical anatomy and physiology and surgical technique related to first assisting
    • Competency in Intraoperative behaviors including handling tissue, providing instruments, suturing and controlling blood loss.
    • Ability to recognize surgical hazards and initiate appropriate corrective and preventative actions.
    • Perform individualized surgical care management before, during and after surgery.
    • Understands and satisfies the needs of the patient population appropriate to the

    age of the patient served (adults, geriatric, adolescent, pediatric, and parents of

    neonates)

    • Certification: BLS must be maintained throughout employment with Terre Haute Regional Hospital
