Outpatient Case Manager (LVN) – Utilization Management- Remote

Working Hours – 9a-6p Central Time
Responsibilities
The LVN Outpatient Case Manager is primarily responsible for providing case management services on specified patient populations. The case manager will assist the concurrent review RN and is responsible for assessing, planning, implementing, monitoring, and evaluating actions required to meet the member’s health and social needs. The case manager will work telephonically with members and will collaborate with providers and other health team members to promote a comprehensive and coordinated approach to addressing members medical needs. This includes but is not limited to: coordination of care for complex cases, collaboration with a multidisciplinary team to facilitate care coordination back into the Kelsey network, conducting client and family education, and conducting follow up calls to members to assist with appointments, discharge needs, and prevent hospital readmissions. The case manager will initiate appropriate referrals to support the member’s health care and social needs.

Salary Range: $53,123 – $65,624 (Pay is based on several factors including but not limited to education, work experience, certifications, etc.)
Qualifications
Education
Required: Graduate of an Licensed Vocational Nurse program; CEU requirement
Preferred: Certification in area of specialization
Experience
Required: 3 years nursing experience as LVN in a physician’s office, clinical, or hospital setting; 2-3 years of experience in case management of high-risk patient population
Preferred: At least 2 years in area of specialization – hospital case management, care coordination and planning, chronic disease management or population health
License(s)
Required: TX LVN license, valid Texas Driver’s license

Required: Excellent communication and problem-solving skills; Proficient computer skills to work efficiently with electronic medical records

Required: Ability to work from designated office space for completion of duties; ability to concentrate in an open-office cubicle environment; ability to operate office equipment such as computer, fax, copy, scan; actively participate in department meetings via phone, computer, or in person. Consistent and prompt attendance at employer designated worksite
