Responsibilities
RESPONSIBILITIES:
Establish rapport and develop a therapeutic relationship with assigned clients. Provides outreach services, information/referral assistance, individual/group treatment services, connects clients to recovery supports.- Maintain appropriate professional boundaries with all persons involved.
- Develop outreach and educational strategies, provide orientation to key stakeholders, develop network support, and formalize partnerships.
- Develop a planning and infrastructure plan to submit to the Department within 90 days of contract implementation.
- Develop Quality improvement and workforce development plan to engage and sustain stakeholders in the community; specifically, primary care, housing, HUD coordinating sites, PATH project staff, mental health providers, employment and vocational partners, etc.
- Participate in evaluation activities included, but not limited to, distributing and collecting date from patient satisfaction surveys.
- Utilize screening tools that are evidence-based or are approved by the Department and coordinate screening assessments with Housing and Urban Development (HUD) and Projects for Assistance in Transition from Homelessness (PATH).
- Provide recovery support services to all clients enrolled in the grant and engage patients in Recovery Support Services.
- Participate in routine reporting of Department and Government Performance and Results Act (GPRA) data.
- Collect and report client intake, discharge and follow-up data through use of the GPRA National Outcomes Measures (NOMS) at intake, discharge and follow-up. Maintain GPRA follow-up rate at 80%.
- Report GPRA NOMS data within 7 days of the intake, discharge, or follow-up interview by entering into SPARS directly or the Department approved system.
- Participate in data requests, interviews and site visits as requested by the Department and the evaluator.
- Report all other substance use disorder treatment data into the appropriate systems as required through licensure standards.
- Provide enrolled patients with a minimum of one recovery support service up to one (1) year per the RSS definitions per the Department approved work plan. Maintain individuals in Substance Use Disorder (SUD) services to maintain eligibility and assure that individuals are enrolled in substance use treatment or other co-occurring disorder service and utilize a minimum of one RSS monthly
- Serves as a case manager for assigned clients.
- Provides crisis intervention and conflict resolution assistance.
- Collaborates with all providers to ensure holistic services are provided. Collaborates with all involved individuals in the provision of care to ensure continuity and maximum effectiveness of care/services provided.
- Develop partnerships and Memorandum of Understanding (MOU) with collateral referral sources.
- Participates in regularly scheduled trainings and care meetings to discuss/update staff on client progress, status, and treatment needs. Professionally communicates with other program staff to promote team collaboration, input, and involvement in problem solving/resolution insuring consistency in the implementation of treatment plan goals/objectives.
- Effectively communicates client service needs with community agencies, schools, courts, service providers of significant others, and mental health/other health care professionals to provide an extended, high quality continuum of care.
- Collaborates with treatment team in developing appropriate discharge plan and aftercare programming. Assists clients in their efforts to locate and obtain appropriate housing post House of Mercy.
Qualifications
QUALIFICATIONS:
High School Diploma or GED required.- Bachelor’s degree in a human services related discipline from an accredited four-year program, preferred.
- CADC Certification within 9 months of hire.
- Valid Driver’s/Chauffeurs License required, must meet Mercy’s Motor Vehicle Safety Standards, must be at least 18 years of age and be eligible to drive per Iowa state law.
- Proof of completion of Mandatory Reporter abuse training specific to population served within thirty (30) days of hire.
- Ability to understand medical terminology and communicate effectively with healthcare professionals.
Pay Range
$17.89 - $24.60 /hour
