Education
Bachelor's degree in a work-related discipline/field (e.g., public health or related behavioral science discipline) from an accredited college or university. Master’s degree preferred.
Experience
Four years of progressively responsible and directly related work experience.

JOB SUMMARY
Program Development, Management and Evaluation
· Defines project scope, goals and deliverables that support program goals in collaboration with management and other stakeholders.
· Facilitates and drives small and large meetings to meet objectives.
· Evaluate effectiveness and appropriateness of program offerings. Develops methodologies for tracking and reporting.
· Disseminate learnings and developments to internal and external stakeholders as appropriate.
· Prepares and maintains accurate, coherent, timely project updates in written and presentation format, including presenting at regional and national conferences and contributing to publishing in academic journals.
Caregiver Support
· Conduct rounds to patient care areas for consultation, coaching, promotion of program goals and a safe environment. Serve as caregiver educator to prepare caregivers for duties performed at home. This role will require 2 days per week at Palo Alto campus.
· Provide empathetic listening, situational guidance, and high-quality information and support to caregivers through one-on-one discussion and attendance at meetings, conferences, and presentations. Assist caregivers in problem solving and understanding disease progression.
· Co-lead caregiver volunteer recruitment, training, and assessment efforts. Mentors and manages team through cycles of continuous improvement.
· Interact with patients, families, providers, and community partners during face-to-face encounters or through a variety of telecommunication strategies in a virtual environment, often establishing long-term relationships. Coordination with Clinical Staff & Community Organizations
· Collaborate with Health Libraries, Supportive Care Program, and other interdisciplinary teams to offer caregiver support and appropriate interventions.
· Conduct community outreach to build and maintain relationships with community-based organizations, county programs, health systems and other partners.
· Represents the program at Stanford and community community engagements. · Collaborates with clinic staff to develop informational/educational materials appropriate for the target population. Assesses learning needs of populations and develops curriculum/content accordingly.

Salary: $100.00 - $140.00 per hour
