Requirements and Qualifications
- Must have 2+ years of experience in health care operations
- Home Health and/or Hospice experience strongly preferred
- Experience with Home Care Home Base preferred
- Ability to market assertively and deal tactfully with customers and the community
Job summary:
- Ensure compliance with all state/federal referral/intake requirements
- Establish/maintain positive working relationships with referral sources
- Coordinate care with multiple service providers
- Provide seamless transition and oversight of patient preparation for home health
- Direct daily patient referral and intake operations efficiently
- Provide the best customer service via phone, email, or in person
- Contribute to identifying opportunities for additional and improved services
