The Division of Substance Abuse of Montefiore Medical Center provides comprehensive substance abuse treatment along with on-site medical and psychiatric healthcare for over 4,000 patients. The program has been commended by state representatives for providing comprehensive services utilizing empirically validated techniques to an underserved population of patients with substance use disorders in the Bronx.
Responsibilities and Duties
We are looking to hire dedicated certified peers to join our innovative substance abuse treatment programs. Job responsibilities include:
· Meet with newly admitted patients to clinics to orient them to the program, inform them of options for expanded care within the programs
· Engage patients in the clinics, particularly those struggling with secondary or tertiary substances in OTP, to consider additional treatment interventions (facilitate referrals to NS/Rising)
· Meet with patients on a regular basis to discuss their care, their recovery, and assist with care coordination
· Outreach patients who have disengaged from care, possible home visits
· Accompany patients to appointments, and assist with care linkages
· Participate in on-site case conference meetings with the clinical team to coordinate care. Participate in agency-wide PAC meetings.
Supervision and training in clinically validated treatment are provided by experienced social workers (LCSWs) or Licensed Mental Health Counselors (LMHC).

Qualifications and Skills
Requirements
· Certified Recovery Peer Advocate (CRPA) will be strongly considered to be part of our Division. Must have an active credential.
· Spanish is preferred
· CASAC credential is a plus.
· Must have knowledge of Medication Assisted Treatment (MAT) and behavioral health conditions.

Pay: $45,000.00 - $53,000.00 per year
