Community-Based Perinatal Patient Navigator
Mission: To pursue equity in maternal health by reducing maternal morbidity and mortality locally, nationally, and globally by building and strengthening community-academic partnerships, developing a competitive translational research program, and offering interdisciplinary and professional training.

The goal of the Community Based Perinatal Patient Navigator (CB-PPN) is to navigate birthing individuals through Georgia’s healthcare system. A Patient Navigator’s role is to help remove any barriers to care the patient may encounter by identifying critical resources for patients, helping them navigate through the healthcare system, and promoting positive health outcomes for patients. The CB-PPN will engage patients, create a trusting relationship, navigate patients to appointments, and closely communicate with a patient’s entire care team.
Minimum Qualifications
- High School Diploma or equivalent required
- Exposure to issues of maternal morbidity and mortality
Preferred Qualifications
- Ability to act as a bridge and educator between patients and their healthcare teams
- Ability to build relationships with different types of people, including patients, organizations, and Care Team members
- Excellent oral and written communication skills
- Flexible and adaptable in response to addressing patients’ and healthcare providers’ needs
- Good communication and interpersonal skills and ability to speak concisely to patients
- Knowledge of maternal health issues in Georgia
- Passionate, trustworthy, and empathetic when working with patients
- Passion for the proper care and well-being of patients
- The ability to provide empathetic care while staying within your scope of practice
- Experience and knowledge of all aspects of the healthcare system
- Ability to provide Culturally Based Communication and Care
- Detail-oriented with the ability to multitask
- Proficient in all Microsoft Applications, including MS Office and Excel
- Strong time management, organizational, and planning skills
- Ability to work evenings and weekends, if needed
