POSITION SUMMARY:

The Community Wellness Advocate Manager is responsible for leadership, supervision and professional development support of CCM CWAs. Specific responsibilities in these areas include performance management, recruitment, onboarding and participation in educational program planning. The Manager may also provide more intensive hands-on support for individual CWAs working with challenging cases, to help the CWA address social barriers and/or partner more effectively with a community partners. Collaboratively with the CCM Director, nurse managers, BH manager, and housing managers, the CWA Manager will participate in CCM program improvement initiatives, bringing unique expertise about social determinants of health and the CWA role to program design and implementation strategy. The Manager also partners with other CCM leaders, the Senior Director, Strategy and Operations, the Executive Director of Growth and Innovation, and BMC Development in building strong relationships with community agencies and resources that support social determinants of health for CCM patients.

ESSENTIAL RESPONSIBILITIES / DUTIES:
  • Supervises a cohort of CWAs, including systematically reviewing their patient panels and the quality of their work and documentation, providing feedback and coaching, and conducting formal performance appraisals.
  • May additionally supervise and provide professional development to a CWA Supervisor(s), who is responsible for performance management of the remaining cohort(s) of CWAs
  • Responsible for recruitment and onboarding of CWAs.
  • In collaboration with other CCM leaders, identifies CWA learning needs and works to meet educational needs of CWAs through educational programming, coaching, and mentoring.
  • Serves as an expert resource to CWAs, CCM interdisciplinary teams and other leaders for consultation, problem solving and ongoing program development.
  • Participates in updating CWA performance standards and competencies insuring practice keeps abreast of local and national advances in CHW practice.
  • Co-facilitates CWA Roundtable.
  • Partners with other CCM leaders, the Senior Director, Strategy and Operations, the Executive Director of Growth and Innovation, and BMC Development, to identify key unmet social barriers for CCM patients, and build strong relationships with community agencies and resources that support social determinants of health for CCM patients.
  • Participates with other staff in activities that include community outreach, presentations to community organizations, development of materials, and phone calls.
  • As an expert in the CWA role and in social determinants of health, is actively engaged with the broader CCM inter-disciplinary leadership team, in discussions, decisions and initiatives to improve the CCM program.

JOB REQUIREMENTS

EDUCATION:
  • HS Diploma with community experiences or Bachelor's degree
  • Bachelor's degree preferred

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
  • Driver's license
  • CHW certification preferred

EXPERIENCE:
  • Minimum of 5 years prior healthcare, public health, or community-based experience in community setting. At least one year of leadership experience in similar setting.

KNOWLEDGE AND SKILLS:
  • Knowledge of healthcare systems and community resources
  • Outstanding interpersonal skills and ability to communicate in a courteous, pleasant and professional manner with direct reports, patients and families, general public, staff members, external agencies, ACO partners, and other organizational leaders.
  • Outstanding ability to communicate and collaborate with other supervisors and managers from other disciplines to solve problems together
  • Ability to give direct feedback and regular coaching to improve performance; ability to mentor, guide and motivate others through demonstration of best practices and leading by example.
  • Extensive experience in community health and outreach.
  • Ability to work independently and to make decisions based on established policies and procedures.
  • Demonstrated familiarity with community health and community health worker (CHW) workforce and landscape.
  • Understanding of the CHW workforce.
  • Demonstrated ability and experience in areas including program development/management, training, team building, problem solving, public policy and advocacy and community networking.
  • Strong education and training skills.
  • Exceptional organizational and time management skills; ability to multi-task and work independently and as part of a team.
  • Excellent oral and written English communication skills.
  • Fluency in Haitian Creole or Spanish preferable.
  • Understanding of how language, culture and socioeconomic circumstances affect health.
  • Desire to work with diverse, multi-cultural and multi-lingual populations.
  • Proficiency with Microsoft Office applications (i.e. MS Word, Excel, Access, Outlook) and web browsers. Proficiency with data entry and data tracking.
  • Ability to provide leadership and coordinate efforts with other team members
  • Demonstrated leadership abilities, prudent judgment, and professional presence and demeanor.
  • Creative problem solver.
  • Ability to adapt to changes in healthcare delivery at local and systems level.

Generally the work schedule will be Monday through Friday. This position may involve work at multiple clinic sites in the Great Boston area. Ability to travel to clinics and community organizations for supervisory and partnership responsibilities will be required when in accordance with public health guidance and BMCHS policy
