Covenant Woods is excited to announce an open position in our Rehabilitation Department!
This program will operate on site at Covenant Woods in Mechanicsville, VA, responsibilities include combination of Healthcare/ Skilled Care and Outpatient Therapy environments.
We are looking for a compassionate, qualified professional who wants to make a difference in the role of Full-Time Physical Therapist.
The Physical Therapist is responsible for implementing and supervising skilled physical therapy services following physicians’ orders and for adherence to all conditions outlined in Personnel Services Agreement.

KEY RESPONSIBILITIES INCLUDE:
  • Provides Physical Therapy Services following written orders according to the physician’s plan of care and defined by the VA Physical Therapy Practice Act.
  • Supervises Physical Therapy Assistants and support staff in accordance with state licensure requirements and professional standards and instruct, supervises, and evaluates home health aide care every two (2) weeks.
  • Responsible for the effective and efficient delivery of rehab services; adherence to an established plan of care, department and patient(s)’ schedule, and timely documentation of services.
  • Attends and contributes to staffing conferences, Organization Home care Members meeting, departmental staffing conferences, family care plans/meeting, Organization In-service Education Programs, and other required meetings.
  • Maintains necessary medical records, evaluation, recertification, clinical notes, discharge notes and conference, incorporated in the patient’s clinical record and completing timely documentation for all patient’s treatment performed.
  • Participates in peer consultation process.
  • Provide in service education programs for nursing organization personnel, as needed.
  • Performs other related duties as required.

QUALIFICATIONS
  • Graduate of an accredited Physical Therapy Program with either bachelor’s degree in Physical Therapy plus 5 years of PT experience,
  • OR Master’s degree in Physical Therapy
  • Two years of appropriate experience as a Physical Therapist.
  • Licensed to practice as Physical Therapist within the state of practice
  • Possess and maintain current CPR certification.
  • Community/home health experience is preferred.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
  • Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy that is certified by the American Therapy Association.
