Lutheran Home is seeking a Physical Therapist to join our team! The Physical Therapist will work with our care team to provide rehabilitation services for individuals in our rehab center and those living in long term care and assisted living. Interested candidates should be motivated, goal-oriented and dedicated to our mission and "4 C's."

We currently have a FT opening and a PRN opening. Hours for the full-time position are Monday - Friday, generally an early morning to afternoon shift, including a rotating Sunday on-call shift.

Essential Functions of the Position:

• Conduct and document initial patient evaluation
• Contribute to care planning, setting objective and measurable goals
• Applying appropriate evidence-based interventions in line with care plan and goals
• Evaluate patient response to treatment and report to interdisciplinary team
• Document ongoing treatment, make recommendations for contented treatment as necessary
• Supervision of PTA/Students as applicable
• Participate in team meetings, contribute to process/quality improvement

Requirements for the position:

• Bachelor's degree in Physical Therapy
• Active state licensure required
• Prior experience working with the adult and/or aging population preferred
• Ability to develop and nurture professional and educational relationships with peers and clients
• Familiarity with Microsoft Office
