We are Utah’s largest Home Based Primary Care 9yrs in a row and counting and we're looking a Full Time MD to join us in Utah County!!

The primary role will be for the practitioner to provide PCP services at the patients home (whether that be at an assisted living facility (AL) or in the patients residential home). Visits will not take place in a traditional clinic setting. The MD will be a mobile PCP and will have the support of additional services such as a medical assistant that works alongside you, office staff, mobile imaging, mobile labs, etc...

Duties to include but not limited to:

• Conducts medical evaluations of predominantly elderly, frail, chronically ill homebound patients in their home environment.
• Documents visit via EMR.
• Develops a treatment plan in collaboration with the patient and family and their goals of care.
• Evaluates patient medication and writes/orders prescriptions as needed.
• Orders and interprets diagnostic tests relevant to patient’s needs.
• Collaborates with multidisciplinary team members.
• Facilitates the patient’s transition within and between health care settings.
• Educates and counsels patients and their families regarding health care options.
• Makes appropriate referrals to home health or hospice as needed.
• Facilitates access to appropriate supportive care services.
• Serves and protects the health care provider practice by adhering to professional standards, policies and procedures, federal, state, and local requirements.
• Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
• Seeing 6-8 patients per day.
• Rotating On-Call schedule (Phone triage after hours) 1 week on call, rotating once every 12 weeks between providers.

Job Type: Full-time

Pay: $120,000.00 - $150,000.00 per year
