Physician - Palliative/Hospice Care
***Sign-on Bonus of $6,000***
Schedule: Full-time, Monday-Friday, 8am-5pm.
On-call responsibilities: Our physicians are on-call one day per week on their regular schedule to assist NPs and RNs with patient facing needs, and they rotate the weekend on-call schedule every 6 weeks.
JOB SUMMARY
The Team Physician serves as an integral member of the interdisciplinary team by providing medical oversight to the care of hospice and home health patients and their families.
This role will provide clinical support to the interdisciplinary team, as well as, provides educational support.
The team physician will assist in educating the broader medical community about hospice and palliative home care and actively network with the community about end-of-life issues.
An active and current medical license to practice medicine in the state of Colorado required before starting work
EDUCATION/EXPERIENCE:
- Board certified in hospice and palliative medicine preferred
- At least two years practice experience required
- Knowledgeable of and adheres to Medicare, Medicaid and hospice and home health standards of practice
- Controlled substance license with current DEA license number
- Eligible or currently possess a Medicare/Medicaid UPIN number
- Prior experience in hospice and palliative care
- License to practice medicine in the state of Colorado
- Privilege eligibility in area hospitals
- Must have professional liability insurance and TRU will reimburse the premium costs
- CPR Certification
- Must possess a valid Colorado driver’s license, provide proof of insurance (a copy of your “Declarations page” indicating you have $100k/$300k Bodily Injury $100k Property Damage coverage) and have reliable transportation
- Must successfully pass agency-required background checks and drug screening
- Must comply with agency health screening and immunization requirements
- Ability to type and/or dictate charting and billing into EHR and complete legibly in a timely manner for patient co-management with other physicians/midlevel providers, nurses and other care plan team members
PRIMARY DUTIES AND RESPONSIBILITIES
- Provides medical consultation to all hospice staff and to attending physicians as requested to review appropriateness of referrals/admissions and care consistent with hospice and palliative care philosophy.
- Oversees medical services for patients on the hospice census, taking responsibility, in the absence of a primary physician, for primary medical management of patients.
- Participates actively in interdisciplinary team conferences, helping to develop and revise patient care plans and acting as a medical resource in determining approaches to individual patient care.
- Works with other staff physicians to assure coverage for IDT meetings and patient care visits when colleagues are on paid time off.
- Assures appropriate utilization of palliative hospice treatments based on time-limited plans, i.e., when intent is to treat symptoms.
- Makes home visits, wherever the patient’s place of residence may be, to provide patient/family medical support and consultations to attending physicians and the interdisciplinary team.
- Works with the Medical Director to provide physician services as needed at the Care Center.
- Takes responsibility for helping to establish appropriate clinical policies and procedures, including pain and symptom control protocols for hospice and home care patients.
- Shares on-call responsibilities with other physicians and nurse practitioners to assure availability of medical services 24 hours a day, 7 days a week.
- Participates in palliative care consult program.
- Establishes and monitors discharge protocols and takes lead in managing decisions for discharge.
- Authorizes as appropriate transfer of patients to the appropriate focus of care within the hospice program (i.e., transfer to care center, change to GIP, routine or respite status, call for continuous care).
- Works in a safety conscious manner which insures that safe work practices are used in order not to pose a risk to self or others.
- Utilizes Standard Precautions in clinical practice.
- Supports TRU Community Care in the conduct of the Quality Assurance/Performance Improvement Program by sharing information and/or participating in quality initiatives and performance improvement activities.
- Assists in the development of in-service education programs for hospice and home care staff and other health care entities in the community and participates upon request in volunteer training and staff orientation.
- Understands and assures compliance with regulations and standards pertinent to state and federal requirements and Medicare/Medicaid hospice and home health benefits and attends regulatory meetings such as administrative law judge hearings as requested.
- Attends regularly scheduled medical services meetings.
- Supervise and teach rotating PA or medical students in a preceptor role shared with other team physicians/NPs.
