Job Description
PURPOSE OF POSITION:
Under general direction this physician line is responsible for carrying out clinical and administrative tasks for WTC Survivor members at the WTC Gouverneur clinic site. Specifically, the physician will provide six (6) clinical sessions per week of direct patient care and two sessions of administrative time per week in the World Trade Center Clinic. The predominant tasks are conducting initial screening evaluations and completing certification requests, conducting periodic monitoring exams, and treating members who have certified WTC-related conditions.
AREAS OF RESPONSIBILITIES
1.Perform WTC initial and annual monitoring exams for WTC EHC members.
2.Submit timely certification requests to NIOSH for WTC related conditions and WTC medically associated conditions.
3.Analyze lab results and provide timely follow up on abnormal results to WTC EHC members.
4.Collaborate when needed with Clinical Director in cancer program case management and prior authorizations and NCCN guideline reviews.
5.Maintain patient/member's WTC records and documentation in Epic/H20, participate in chart review for preauthorization for tests/procedures/admissions when needed.
6.Provide treatment to WTC Program patients for WTC covered conditions including but not limited to aero digestive diseases and psychological and social problems. Actively participate in our Collaborative Care Model of healthcare delivery.
7.Communicate with internal staff such as care managers and other external providers involved in WTC EHC member's WTC-related plan of care.
8.Participate in medical and departmental staff meetings, WTC program research activities and team educational programs necessary for compliance with the WTC Program, maintain Citi IRB research training and Good Clinical Practice training competency.
9.Perform quality review on designated measures as directed by Medical Director.
10.Follows Gotham, NYC HHC and facility-specific policies and protocols (including but not limited to policies around DNKA, Normal Notification, call-outs, dress code, and corporate job descriptions).
11.Meets all compliance standards and requirements (including but not limited to annual physical assessment, fraud and HIPPA trainings, Fire Drill participation).
12.Ensures that credentials are maintained.
13.Maintains Patient Safety and Infection Control Protocol as per Network and Departmental Policies.
14.Upholds environmental safety standards to assure safety of patients and staff.
15.Fosters/exercises Service Excellence in daily activities, when dealing with general public and staff.
16.Performs administrative, professional and consulting duties as requested by the Chief of Service, Medical Director, or designee.
17.Performs other duties as needed.

SUMMARY OF DUTIES AND RESPONSIBILITIES
1.Evaluate and manage patients with chronic medical conditions. This will include routine visits for these conditions, taking interval histories, performing appropriate and focused physical examination, ordering necessary blood work and radiology studies, following up on the results of these studies, adjusting existing medications and initiating new medications as necessary, and arranging appropriate follow up with him/herself.

Minimum Qualifications
1. Graduation from an approved Medical School; and,
2. Licensed to practice in New York State; and,
3. Completion of an approved residency in the specialty and Board Certified or Eligible (if Eligible, must obtain certification within five years of eligibility); and,
4. Must meet the criteria for credentialing and privileging in the department as defined by the Medical Staff By-Laws of both the facility employing the Physician and the back-up hospital, if these are different.
5. Must meet all NYS Medicaid Managed Care requirements.
