The Chronic Disease Program Manager will work with the NARA health team to plan, implement and evaluate effective and culturally appropriate chronic disease prevention and management services for AI/AN and other underserved people in the greater Portland metropolitan area. This position will be responsible for supervising ten full-time people, including staff of the Breast and Cervical Cancer Early Detection Program (BCCP), Diabetes Treatment & Prevention Program, the WISEWOMAN Program and the Colorectal Cancer Early Detection Program. This position will assist the NARA Chronic Disease Program Director in reporting and day to day grant requirements of the public health/chronic disease initiatives funded through various federal and state agencies. Coordinating with other health and wellness programs, the Program Manager will also represent NARA at community and partnering events. A key focus will be in building relationships with tribal agencies and tribal clinics as well as community partners that provide service to the uninsured and or underserved populations. This position will also assist staff of the Chronic Disease program in meeting its service delivery goals by overseeing the recruitment of eligible clients and the data management for program and patient services.

ESSENTIAL JOB DUTIES:

  • Supervise ten full-time staff. Review and approve timecards, conduct performance reviews, provide timely coaching and feedback to employees, and identify personal growth opportunities for all employees. Cross-train staff to strengthen cohesiveness of Chronic Disease Team.
  • Recruit, orient, and train new employees.
  • Administer chronic disease grants- NBCCEDP, CRCCP, SDPI, HRSA NHCI and WISEWOMAN (total of 1.8 million dollars/year) to ensure programs are implemented with fidelity to all grant requirements. Interpret state and federal grant statutes, rules and regulations and integrate them into contracts and/or program policy and procedure manuals.
  • Work with Program Director to establish contracts for grant related health services, initiate contract amendments, track budget expenditures, reallocate funds as needed
  • Oversee evaluation quality assurance activities including working with the Chronic Disease Program Evaluator (NARA Evaluation Team) to create and implement a process for collecting data/information required to track and report on program performance measures.
  • Submit narrative and data reports as required and upon request
  • Work with Program Medical Advisors (CMO, Deputy CMO, Primary Care Providers) and Program Director to provide clinical oversight to breast and cervical cancer, colorectal cancer, diabetes and cardiovascular disease programs.
  • Work with Public Health Program Consultant who will assist with development of policy and procedure manuals as appropriate and review the final documents to ensure clinical information is current and accurate.
  • Help coordinate and participate in relevant trainings to ensure staff can serve as subject matter experts in their field.
  • Educate internal and external partners on those subjects and identify potential collaboration efforts
  • Develop and update communications in partnership with the Program Coordinators and the NARA NW Development Team to inform the public of available services.
  • Develop, revise, and report on project goals and priorities. Follow up with internal and external health systems partners to ensure tasks are completed and deliverables are achieved. Provide support and guidance in identifying program needs and implementing plans to improve program quality and efficiency. Establish and maintain effective working relations with local, state, federal and tribal agencies to facilitate improvements in program planning and resource utilization.
  • Perform other duties as assigned.

QUALIFICATIONS:

  • Bachelor’s Degree in a health related field and four years of professional experience managing health programs and supervising health program staff. Any satisfactory combination of experience and training which demonstrates recurring knowledge/skill and ability to perform above-described duties.
  • Evidence of computer proficiency in Microsoft Office Suite (Word, Excel, Access and PowerPoint); Publisher, Electronic Health Records, and database management, experience with Med-IT and Athena Practice a plus.
  • Availability to work some evenings, weekends and special events, as needed.
  • Availability to travel to conferences/trainings as needed.
  • Evidence of ability to convey information clearly, effectively and in grammatically accurate way.
  • Evidence of ability to gather information and apply it to current situation in order to arrive at conclusion, solve problem or make a valid decision.
  • Ability to maintain professional boundaries with clients, the community, and other staff members.
  • Work collaboratively with clients, other NARA staff, team members, and to treat everyone with respect and dignity at all times.
  • Experience working within the Native American/Alaska Native community is considered a plus.
  • Ability to travel between multiple sites and locations in a day.
  • Ability to pass a pre-employment or for cause drug tests.
  • Ability to pass criminal background and DMV checks.
