Rehabilitation Coordinator

Qualifications:

1. BA/BS degree in human services, psychology, or social services with at least 1 year of experience working with clients with severe emotional and psychiatric disorders.

2. Or a high school diploma with some coursework in human services, psychology, or social services with at least 3 years of experience in providing therapeutic or treatment services to clients with severe emotional and psychiatric disorders.

3. Candidates must show documented experience in planning therapeutic, recreational, or treatment activities for clients with physical, emotional, or medical disability for clients on caseload.

Responsibilities:

1. Teach clients independent living and community integration skills.

2. Provide supportive counselling to severely emotionally distressed clients on caseload.

3. Provide progress notes, document interventions, and summary of activities/ interactions with clients.

4. Review and coordinate the collection for relevant information on referred clients background, social history, and past treatment interventions in order to develop treatment plans per client.

5. Evaluate, monitor and review individualize treatment plans and assess individual progress in treatment and make recommendation for modification, changes, or termination of treatment.

6. Maintain records as required by established laws, policies, and administrative regulations including COMAR regulations.

7. Attend all discharge, termination, admission, and clinical supervision as scheduled to promote client services or professional development.

8. May act as a consultant to outside providers, school personnel, and medical staff on the individual needs of clients on caseload.

9. Collaborate with all relevant agencies to support client’s overall wellbeing.

Reporting authority: Must be supervised by the Rehabilitation Specialist and Program Director.

Pay: $20.00 - $25.00 per hour
