Job description
Major/Essential FunctionsWork performed as part of a total health care team in providing patient referrals coordination and navigation including:
Tracking and documentation of patient referrals to promote team awareness and ensure patient safety
Assemble patient demographic, insurance/payer, and health data
Insurance/payer prior authorization
Provide patient and family communication, navigation, and advocacy through the referral process
Establish and maintain communication and relationship with identified service providers and community resources
Update patient records with demographic, insurance, and information on specialist
consults, hospitalizations, ER visits, and community organizations related to their health.
Required Qualifications
High school graduation plus five (5) years of progressively responsible related experience; additional related education may exchange for the required experience on a year-for-year basis.

Medical Assistant certification

Effective verbal and written communication skills
Teamwork orientation
Organized and able to manage competing priorities
Resourcefulness in problem-solving
Able to take and follow through with delegated tasks and accountability
Pay Statement
