East Valley Territory: Mesa, Gilbert, Apache Junction, Gold Canyon, San Tan Valley, Queen Creek, Chandler, Sun Lakes.

Benefits:

• $50-$70 per visit (Non-Medicare)

Responsibilities:

• Provide thoughtful and quality home carProvide full nursing care including but not limited to; IV (PIC, TAC, PORT PICC). wound care, medication management

• Monitor and track changes in the client/patient’s conditions and needs.

• Document and update the Director of Nursing

• Proactively updates client/patients’ family on plan of care, health changes, and concerns

Requirements:

• Required to have a minimum one (1) year of documented experience in a home health agency

• Current unrestricted RN in Arizona (Prefer Bachelor’s Degree in Nursing)

• Current Negative TB test

• Ability to secure Fingerprint Clearance Card

• Participate in the On-Call Rotation
