Full time, 80 Hours Biweekly, 8:30 am to 5:00 pm

Qualifications

Current PA Professional Registered Nurse License.

Minimum of 5-years’ experience working with management.

Job description

1. Performs outreach, including but not limited to PA Health & Wellness, Centene, providers, counties, state officials, as needed, and any applicable outside stakeholders to increase awareness of the company and services offered.

2. Understand the process of the development of appropriate individualized care plans based on age, cultural and spiritual values.

3. Participate in the development, revision and implementation of policies and procedures designed to insure comfort and safety of participants in collaboration with other health care personnel.

4. Ensure that participants receive quality care and that they are satisfied with the services they receive. Resolves client complaints and problems, documents issues, and enters incident reports into Enterprise Incident Management (EIM) or other critical incident management system as necessary.

5. Facilitate a team-based approach to program supports and services while practicing open communication and respecting diversity.

6. Complete all required reports and documentation.

7. Provide training and orientation to new team members; Develops appropriate care plans for participants; Monitors compliance with rules, policies, and regulations; Manages time tracking and approves time-off; Communicates expectations and delivers constructive feedback.

8. Keep abreast of relevant trends, rules, and regulations in the industry. Makes recommendations to management for changes and improvements to enhance services or efficiency. Communicates required changes to leadership teams.

9. Respect the autonomy and right to confidentiality of all participants and families.

10. Perform all duties in an ethical manner and in compliance with local, State and Federal laws, Medicare/Medicaid regulations and established personnel policies and procedures, including confidentiality and HIPAA guidelines.

11. Complete weekly audits for all locations around and track data from audits.

12. Analyze data from audits and implement new strategies and processes to increase proficiency of compliance.

13. Respond effectively in emergency situations so that the client’s immediate needs are addressed.

14. Respond to emergency calls if/when they occur to ensure the safety of the individual. Provides coverage in emergency and/or crisis situations.

15. Monitor patients’ progress and service delivery through on-site visits and data collection.

16. Travel to and participates in monthly team meetings, ISP meetings, Annual Review Updates, and other team meetings, as necessary.

17. Facilitating meetings with teams that are not meeting quality compliance standards.

18. .Collaborate with all members of OLTL Team to develop, aggregate and maintain policies, procedures and activities pertaining to current and new OLTL Regulations put forth.

19. Maintain program compliance with all applicable state funding agency and certification regulations and requirements, as well as quality service standards.

20. Communicates regularly with management, providing status and activity reports as agreed. Participates in Keeps abreast of relevant trends, rules, and regulations in the industry. Makes recommendations to management for changes and improvements to enhance services or efficiency. Communicates required changes to Directors, Managers, and Program Specialists

21. When assisting new team members – oversee all personnel activities, including the trends and issues regarding hiring, orientation, training, supervision and evaluation of program employees and consultants.

22. Facilitate a team-based approach to program supports and services, practice open communication and respect diversity.

23. Ensures quarterly and annual audits are completed and results reported.

24. Implements new QMP goals if a goal is completed.

25. Complete the client assessments and all corresponding documentation when necessary.

26. Document assessment/reassessment, plan, interventions, and outcomes.

27. Knowledgeable in abuse recognition and referral. Safeguard the patient from incompetent, abusive or illegal practice of any individual.

28. Perform other job-related responsibilities as assigned.
