PART TIME REGISTERED NURSE & EXPERIENCED LPN (PER DIEM) - HOME HEALTHCARE AGENCY

Home Healthcare Agency seeks experienced and flexible Registered Nurse ("RNs") and LPNs with case management experience to staff a 24/7 tracheotomy case on a Per Diem basis. This position is ideal for qualified RNs/ LPNs with Trach, Vent and Tube Feeding experience seeking flexible work or incremental hours. Available shifts are 4/8/12 hour blocks with overnight and weekend options available. We offer competitive hourly pay and limited travel – service calls are in private home in West Philadelphia. PER DIEM REGISTERED NURSE/ LICENSED PRACTICAL NURSE RESPONSIBILITIES:

• Must be reliable with requisite experience and willing to take private duty shifts in 4/8/12 shift combinations including overnights and weekends.
• Must possess strong case management skills and experience with Adult Trach, Vent and Tube Feeding
• Administrative experience with tasks such as overseeing the maintenance of client clinical records, statistics, reports, and records for purposes of evaluation and reporting of organization activities (*Preferred)
• Occasionally supports clinical and field service team by supervising, educating, and coaching care-giving field staff.

REQUIREMENTS

• Current and unrestricted state of Pennsylvania Registered Nursing or LPN license
• Candidates should ideally have minimum of three (3) years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility.
• Bachelor’s degree in Nursing or LPN accreditation /must have graduated from a National League of Nursing accredited program
• Excellent customer service skills and ability to deal tactfully with clients, staff and community.
• Clean background and criminal record
• Adhere to HIPAA and maintain client confidentiality
• Demonstrates understanding of, and sensitivity to cultural and religious beliefs in plan of care/education.
