Join the Bestcare Home Health Care Team!

• We are seeking Per Diem and Full Time Field RN's to work with our clients in Manhattan, Brooklyn, Queens, The Bronx and Staten Island. Also, Nassau , Suffolk and Westchester Counties. Flexible Per Diem Hours.

Our corporate Office is located at Levittown, NY (Nassau County)*

Field RN Job Responsibilities:

· Conduct initial home visits to assess patient needs and develop the Aide Plan of Care

· Communicate with a patient’s physician and other team members to ensure delivery of appropriate patient care

· Supervise patient care provided by our Home Health and Personal Care Aides

· Responsible for the review and maintenance of patient records, using telehealth modalities for early intervention of patient issues

Job Qualifications:

· Must be a valid Registered Nurse licensed in New York State, preferably with two (2) years of experience

· Must have a valid driver’s license and transportation (other than Manhattan). Monthly Metrocards will be provided for RNs at the Manhattan location

· Previous home care experience is a plus

· Bilingual Spanish and English language abilities is a plus
