JOB DESCRIPTION

POSITION TITLE: Registered Nurse

POSITION SUMMARY: A Registered Nurse designated to perform Participant health assessments; provides training/oversight to staff for medication assistance, health monitoring and assistance with health-related tasks; coordinates Participant care with third-party providers such as physicians, specialists, therapists.

REPONSIBILITIES:

  • Adheres to and conveys the philosophy of supporting the dignity, privacy, independence, choice, and individuality of Participants.
  • Provides training and oversight to licensed and unlicensed personnel in the provision of services as allowed by regulation.
  • Coordinates Infection Control program- Provides training and oversight to ensure appropriate infection control procedures are followed by staff.
  • Develops implements and maintains written objectives, standards of practice, policies and procedures and the organizational plan for the nursing department.
  • Conducts regular quality assurance reviews of staff documentation of medication/treatment assistance provided.
  • Interviews Participants and families in order to determine healthcare and support needs. Conducts Participant health assessments with new Participants on the first day of participation and formulates a plan of care within five business days of their start dates, the comprehensive assessment must be completed within fourteen days.
  • In conjunction with the Nursing department, coordination with Participant physicians and other health care providers received at the facility, at home and in the community as needed.
  • Provides coordination with the pharmacies used by the facility and Participants. Coordinates and oversees the implementation of all new physician orders.
  • Monitors the status of Participant health conditions and initiates actions in response to changing needs; conducts re-assessments of Participant health needs as indicated.
  • May participate in the pre move-in Participant assessment/ certification, in the participant’s home environment, process as requested by the Administrator.
  • Serves as a medical resource for the program as a whole.
  • Assists the Administrator with conducting and/or coordinating staff in-services and training, as requested.
  • Responds and advocates for Participant, family or staff concerns and reports them to the Administrator and/or others as appropriate.
  • Provides health counseling/education to Participants as needed.
  • Performs TB tests for Participants and staff as needed.
  • Reports any abuse, suspected abuse, or alleged abuse per state regulation and facility policy.
  • Develops community medical referral resources and maintains ongoing communication with them,
  • Performs all other duties as assigned.
  • Serves as designated alternate DON in the absence of the DON.
  • Participates with the Interdisciplinary care team.

QUALIFICATIONS:

  • Desire to work with the elderly in an environment that upholds their privacy, dignity, choice, independence and individuality.
  • Working knowledge of those principles necessary to provide competent nursing care.
  • Understanding of the normal dosages, actions and common side effects of most medications in common use.
  • Good oral and written communication skills and the ability work well with others.
  • Effective training and supervisory skills. A registered professional nurse who has at least one year of full-time experience in nursing supervision and/or administration in a licensed health care facility.
  • The ability to identify, address and communicate problems effectively.
  • Working knowledge of and ability to conform to applicable laws, rules and regulations.
  • All general staffing requirements:
  • Free of communicable disease
  • Able to physically/mentally perform tasks, as verified by a health screening that includes TB clearance
  • Criminal record clearance
  • Current First-Aid certification (CPR)
  • Ability to read, write and understand English
  • General understanding of the needs of the elderly
  • Willingness to learn

EDUCATION AND EXPERIENCE:

Must be at least 21 years of age. Must have a current RN license for the State of New Jersey and be a graduate from an accredited school of professional nursing. At least one year experience working with the elderly in a licensed Healthcare Facility.
