Position Objective:

The Nurse will provide in clinic support services for child and adolescent and adult consumers of Health Connect America, Inc. These services will be targeted at providing nursing services as well as facilitating telehealth appointments. This will be approximately 20 hours weekly.

Essential Duties:

• Travel to the Dallas and Tyrone locations to provide nursing services (one day at each location per week).
• Set-up telehealth equipment at identified telehealth sites.
• Collect vital signs and necessary medical information as ordered by the telehealth physician.
• Assist with the scheduling process.
• Document the provision of services.

• Perform nursing assessments and complete associated documentation. These assessments include obtaining clients’ medical histories, vital signs, a brief mental status exam and information on the effectiveness of psychotropic medication and any side effects.
• Perform DATEP nursing assessments and complete associated documentation on clients where indicated as needed by intake for clients with substance abuse history.
• Perform nursing re-checks or follow-ups. The Nurse will have follow-up appointments with clients to assure medication effectiveness and to check for medication compliance and side effects. Updating physical medical problems will be included.
• Contact the prescriber if information is gained at the assessment or follow-up that requires the doctor’s intervention, such as changing medications or dosages and referral to a higher level of care in cases of emergency.

• Dispense medication by the psychiatrist’s order from the stored sample medicines. On occasion, this will include giving injections of psychotropic medication.
• Complete medication authorization referral for any medications that require this process, including insurance authorizations and referral to medication assistance programs.
• Participate in integrated care efforts.
• Provide education to clients regarding their mental and medical health (i.e. smoking cessation, nutrition information etc).
• Maintain library of nursing educational materials.
• Other duties as assigned or requested by the supervisor or telehealth physician.

Knowledge, Skills and Abilities Required:

• Licensure by the State of Georgia as a Registered Nurse (RN).
• Experience in a psychiatric setting is preferred and a general knowledge of mental health disorders necessary.
• Good communication skills, i.e. ability to listen and ask pertinent questions to gain needed information.
• Computer skills adequate for operating telehealth equipment, inputting consumer information, creating cases, scheduling appointments, and e-mailing and documentation.
• A knowledge of an practice of the Health Connect America’s policies on ethical standards, confidentiality, HIPAA regulations and an ability to provide comprehensive and thorough documentation of case notes.
• Contribute to Health Connect America culture of safety and inclusion by demonstrating a commitment to learning how to engage with issues of diversity, equity, and inclusion.

Physical Requirements:

• Ability to interact with consumers and referral sources on phone and in person for possibly extended periods of time.
• Ability to operate a vehicle.

Working Conditions:

The Nurse will provide services in the community and at Health Connect America offices at times. Staff must use personal transportation to perform essential functions of the job.

Computer, computer programs, internet, telephone, screening forms, personal transportation, telehealth equipment.
