RN Home Health Case Managers- Flexible Schedules!

Home Health experience is NOT required, we offer an extensive Home Health orientation program!

VNA Care is seeking talented RN Home Health Case Managers to join an amazing team based out of VNA Care’s Worcester office.

Roles available for:

• Team 3: Shrewsbury, Millbury, Sutton, Douglas, Worcester
• Team 4: Rutland, Paxton, Holden, W. Bolyston, Boylston, Worcester

RN Home Health Case Managers work under the supervision of a Clinical Manager and provide skilled nursing care in patient’s homes in a designated territory. Effectively manage patient specific health concerns such as: COPD, diabetes, heart disease, stroke, cancer and complex wounds.

Schedule:

Monday thru Friday 8AM- 4:30PM. Flexible start times are available.

How We Know You're A Good Fit:

• Must have good standing license (RN) in the state of Massachusetts.
• Must be willing to drive within a territory.
• Minimum of 1 years general med/surg or acute care experience.
• Demonstrates a can-do attitude and the ability to respond appropriately to the needs of patients and families in varied environments.
• Proficient interpersonal and communication skills.
