Assessment Nurse (RN)

Schedule: Fulltime - Monday - Friday 9:00am - 5:00 pm

Pay Rate: $42
* 1 week paid orientation provided
Summary : Registered Nurse (RN) provides in-home health assessments and services to geriatric & hospice patients, identify goals to keep patients in the home safely by
putting in place repairs, home modification and/or other equipment that patient may need to be safe at home. RN will work as a team with an OT & Handy man. PPE Provided.
Requirements

Associated or Bachelor Degree in Nursing

NYS Registered Nurse
- CPR/BLS Certificate
- Proof of Covid-19 Vaccination

1-2+ years of experience in homecare or palliative care required

Must be available to work Fulltime Monday - Friday for a Minimum of 12 weeks.

Bilingual a Plus
Responsibilities:

Assess patients condition and chart your observations

Perform evaluation tasks, such as vital signs and administering medication as prescribed by Physician

Dress or redress open wounds and assess progress of healing. May include: wound vac, diabetes monitoring, infusion therapies and phlebotomy

Educate patients and their families on proper home health care strategies and procedures

Coordinate with Occupational Therapists, Physical Therapists, Physicians in the patient’s care plan

Make recommendations for devices or tools that might improve the patient quality of life
