Social Services
:
planning and implementing a program that offers counseling and other support services for residents and their families
evaluating, determining and creating a plan base on the medical-related social needs of the residents
coordinating and implementing family meetings for residents to keep the communicatin flowing
acting as the liasion between all parties
coordinating discharge planning
Qualifications
For Social Services
:
- High School Diploma or equivalent
- LSW/MSW
- Previous LTC/SNF experience preferred
- Serious applicants
- Interest in the geriatric population
