• providing case management services, inform participants and their families/caregivers of available community services and make referrals as needed.
• The Social Worker will also be responsible to develop and lead groups for the participants.
• Assist the Program Coordinator in referral, admission and discharge process.
• Write progress notes quarterly.

• Attend monthly supervisor meeting with the Program Coordinator (bi-weekly with LICSW if not licensed).
• Assist, as necessary, in the delivery of other required program services.
• Participate in the bi-annual or quarterly review of each participant's care plan as well as assisting with the development of care plans.
• Assist Program Coordinator with the implementation of policies and procedures.
• Assist Program Coordinator with record reviews, documentation, compliance and implementation of policy and procedure.
• Ability to provide services outlined in this job description at any of the three Adult Day health sites as assigned by the Program Coordinator.
• Respond to reasonable expectations of the Agency and/or supervisor.

• A Bachelor's Degree is required along with one year of recent experience working with disabled/elderly adults in a professional capacity is required.
