Good Shepherd Hospice is seeking a compassionate Social Worker to serve our patients and their families in the* LEBANON, MO and surrounding communities.*

The Hospice Social Worker provides medical social services to patients and their families, working as a member of the interdisciplinary hospice team.

Key Responsibilities

• Assessing Significant Social and Emotional Factors for Patients and Families
• Helping Patients and Family to Understand, Accept and Follow Care Plan
• Use All Available Means to Help Patients and Family To Live Better Within Illness Limitations
• Participates in Ongoing Educational Programs and Special Projects
• Supports Staff with Loss and Patient/Family Issues

Skills Knowledge and Expertise

• State Certification
• Masters Degree in Social Work from an accredited university
• Works as a team player, following directions and adhering to defined care plans
• Demonstrated commitment to the hospice philosophy of care
• Can successfully pass background checks
• Valid Driver's License
