Good Shepherd Hospice is seeking a compassionate PRN Hospice Social Worker to serve our TULSA and surrounding communities. PRN Hospice Social Worker provides medical social services to patients and their families, working as a member of the interdisciplinary hospice team.
Key Responsibilities
- Assessing Significant Social and Emotional Factors for Patients and Families
- Helping Patient and Family to Understand, Accept and Follow Care Plan
- Use All Available Means to Help Patient and Family To Live Better Within Illness Limitations
- Participates in Ongoing Educational Programs and Special Projects
- Supports Staff with Loss and Patient/Family Issues
Skills Knowledge and Expertise
- Masters or Bachelors Degree in Social Work from an accredited university
- Works as a team player, following directions and adhering to defined care plans
- Demonstrated commitment to the hospice philosophy of care
- Can successfully pass background checks
- Valid Driver's License
