JOB SUMMARY

Community Clinical Services, affiliated with St. Mary's Health System, is seeking an organized, self-motivated person to provide case management services to adults with emotional or psychiatric diagnoses as part of the Behavioral Health Home Organization. The BHHO is a team approach to case management and provides improved care coordination and access to services that address both mental and physical health, as a whole-person approach to wellness. This person will function as part of an interdisciplinary team and will be responsible for managing a caseload of patients.

JOB REQUIREMENTS

EDUCATION

• MHRT/C is required
• Bachelor’s Degree in relevant field preferred, but not required
• A valid Maine Driver’s License required-travel is required for community visits

EXPERIENCE

• 1-3 years experience in case management is preferred, but not required.

OTHER SKILLS REQUIRED

• Excellent verbal, written, and interpersonal skills
• Must be able to work efficiently as part of a treatment team
• Requires good problem-solving, organization, and time management skills
