The Social Worker must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans as they transition from the CLC to appropriate placement alternatives and/or are served within the COACH Program. Care management responsibilities include the management, coordination, and provision of social work and supportive services to Veterans and/or their families. The COACH program provides home-based dementia care services to veterans who are frail, with neurocognitive disorders and may present frequent and serious behavioral symptoms, and their primary caregivers who may experience significant stress and coping difficulties with their multiple caregiving challenges. The Social Worker is responsible for ongoing psycho-education and support to maximize Veterans quality of life, assist COACH Veterans to remain in their home for as long as possible, and to mitigate the stresses of experiencing and caring for conditions associated with neurocognitive disorders. The psychosocial services are provided in the Veterans homes, clinic, community, via telephone and telehealth visits.
CLC offers extended care and rehabilitation services to eligible Veterans. The Social Worker in CLC provides social work services to the population of Veterans receiving medical care in the Community Living Center at the Durham VAHCS. The Social Worker demonstrates skills and judgment and has knowledge of practice related to Geriatrics and Long Term Care as well as the growing population of patients with chronic illness and terminal conditions. In collaboration with the multidisciplinary medical team, the social worker evaluates the Veterans presenting psychosocial problems and facilitates access to the treatment and resources that can most effectively minimize the Veterans psychosocial stressors. This includes providing high quality case management, clinical treatment and intervention, advocacy and coordination of care to other appropriate VA or community service providers and/or agencies as needed by the client. By involving and educating families and caregivers, the Social Worker works to optimize the social supports available to the veteran in order to promote maximum recovery and functioning.
Clinical Functions:
- The Social Worker Must have a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans in the Community Living Center and COACH Program and/or their families. The Social Worker is able to independently work with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems.
- Must independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. The Social Worker must be able to utilize this assessment in facilitating the Veterans maximum use of treatment for attainment of the highest level of independence that is possible and practicable.
- Must actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of a resident centered treatment plan.
- Must possess a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, common medications and their effects/side effects, and medical terminology.
- The Social Worker Is responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility.
- Is willing to serve on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor and Chief of Social Work Services.
- Is mindful of keeping their supervisor apprised of problems and recommended solutions to problems encountered in the incumbent's area of responsibility.
- Is responsible for furthering one's own professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment.
Work Schedule: Monday- Friday 8:00a.m.-4:30p.m.

Qualifications
Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
- Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going tohttp://vaww.va.gov/OHRM/T38Hybrid/.
- English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Grade Determinations:
Social Worker, GS-11
Experience and Licensure: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9grade level in the field of health care or other social work-related settings, (VA or non VA experience) and licensure or certification in a state at the independent practice level.
OR
Education: In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate all of the following KSAs:(a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
(b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
(c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
(d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
(e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.

Full Performance Level. This is the full performance level. Social workers at this level are licensed or certified to independently practice social work. Incumbents are assigned to all program areas, including but not limited to: inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area. They independently formulate and implement a treatment plan including measurable, achievable goals identifying the Veterans' needs, strengths, weaknesses, coping skills, and psychosocial acuity. Social workers serve on committees, work groups, and task forces at the facility and VISN level or in the community.

Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. The Social Worker must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the Social Worker or others.

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
