Full time Social Work (MSW) positions with University of Rochester Medical Center. Social worker collaborates with multidisciplinary team members and joins a large team of social workers.

Responsibilities: psychosocial assessments, resource mobilization, and assistance navigating health care system, coordination of patient discharge and planning for after care needs and patient and family support. Currently recruiting for SUMMITS (Substance Use disorders, Medical Management, Infection Treatment and Support) that offers co-management of medical and substance use management to people who have substance use disorders or addictions who need to stay in the hospital for long courses of treatment.

The 886-bed facility boasts a regional trauma and burn center, pioneering transplant programs, esteemed programs for many acute and chronic illnesses.

Requirements: MSW degree with experience preferred and licensure obtained within 18 months of employment. COVID-19 vaccination is a condition of employment.
