Case Manager Social Workers provide direct discharge planning services and assistance with psychosocial services for the patient and family.

Schedule: Three 12 hours shifts from 11:00 AM - 11:300 PM, Rotating Weekends and Holidays

Responsibilities:
Postion Summary:
Case Manager Social Workers provide direct discharge planning services and assistance with psychosocial services for the patient and family.
Collaborate with physicians and other members of the healthcare team to develop a discharge plan in relation to the patient’s health care needs.
Utilize broad experience and knowledge of appropriate healthcare resources for patients of all ages.
Promote continuity through communication of the patient’s plan of care.
Accountable for self and self-actions:
Engages in self-evaluation of practice and takes action to improve knowledge and skills.
Promotes consensus building and applies techniques of conflict resolution and conduct improvement.
Retains accountability for quality of care given to the patient.
Qualifications:
Requirements:
Master of Social Work degree from an accredited facility.
Clinical emphasis from a CSWE accredited graduate school.
State of Kansas licensure at the LMSW level.
Prefer two semesters of graduate field experience in healthcare setting for a new graduate OR previous experience in a healthcare setting for an experienced graduate.
Broad knowledge of healthcare and community resources.
Interpersonally gifted possessing excellent communication and negotiation skills.
Self-motivation, tenacious, and enjoy a diverse, fast-paced work environment.
Computer literacy.
