GENERAL SUMMARY:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care. Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy; Education provided to physicians, patients, families, and caregivers; and Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.

Principal Duties and Responsibilities:
Will be comfortable carrying a caseload of 8-10 patients depending on census
Will be able to work with a broad range of patients with co-occurring mental illness and substance abuse
Will provide patient assessments and diagnosis
Will work with patient to identify treatment plan needs
Will be able to lead groups focused on mental health disorders and substance abuse
Will have knowledge of case management procedures and discharge planning
Will have strong treatment planning and documentation skills
Will have knowledge of insurance requirements and be able to perform utilization reviews
Will be capable of crisis assessments and moving patients to higher levels of care when clinically indicated
Will have knowledge of Massachusetts regulations with regard to duty to warn and protect
Will be compliant with maintaining standards in regards to outside credentialing agencies such as The Joint Commission and insurers

KNOWLEDGE, EXPERIENCE AND OTHER JOB REQUIREMENTS:
Master’s degree in Social Work or Mental Health Counseling is required
Massachusetts license at LICSW/LMHC level is preferred

Experience:
Group therapy experience required
2 years’ experience in substance abuse/dependence treatment preferred
